When you come for your initial appointment, please arrive 25 minutes early to complete any necessary paperwork. New patients should bring the following information.
Important Note: If you are a new patient and you have x-rays, MRI's or CT scans, please bring copies with you or make sure they have been forwarded to our office. Upon arrival new patients will have to complete our new patient information forms.

Insurance & Referrals
Some insurance plans are accepted for selected procedures, office visits or consultations. Please call for more information or if you have any questions concerning your insurance. Our insurance specialists will help you navigate your insurance coverage, file claims and help with pre-authorization for treatment. Patients are required to pay insurance deductibles or co-payments at the time of service.
Some insurance companies require a referral for an appointment from your primary care provider or other referring physician. If you have any questions, please contact your primary care physician or call our office for confirmation. You may be seen only for what is recommended by your doctor and/or the referring agency.
In addition to cash and checks, we accept Visa, MasterCard, Discover and American Express. Please call our office if you have any questions about insurance or to make payment arrangements.
If you need a prescription refilled before you next appointment, please call our office to make arrangements. When calling, be sure to have ready all prescription information and the phone number of your pharmacy. Not all prescriptions can be filled the same day, so please allow at least 48 hours for a prescription refill. Our prescription number is .
Medical records can be transferred to another physician or medical practice with a proper written request from the patient.
